The Department of Higher Education (DHE) encourages you to use this form to make a public records request (PRR). This helps to ensure the most expeditious and accurate response to your request.
Written requests may also be delivered by hand, mail, email or fax (if available) or by telephone and orally in person to a Records Access Officer (RAO). Please note, however, that requests made orally or by telephone may not be the subject of an appeal.

Only questions pertaining to the PRR process or a PRR status should be sent to the email account of the Primary RAO. All other questions for the Department of Higher Education should be submitted using the general email form.

The DHE does not hold student records for education verification. Please contact the institution directly for that information. Voice messages and emails that request education verifications will not be returned.
